Learn how to apply for your card.<\/p>\n<a href=\"\/patients\/cards\/\" class=\"wm-button size-medium button\">learn more<\/a>\n\t<h3>Log into the Registry<\/h3>\n<p>In the Medical Marijuana Use Registry, patients can apply for an MMUR Identification Card and view the orders their qualified physician placed for them.<\/p>\n<p>&nbsp;<\/p>\n<a href=\"https:\/\/mmuregistry.flhealth.gov\/Public\/LogIn\" class=\"wm-button size-medium button\" target=\"_blank\">Log in<\/a>\n\t<h2>Steps to Treatment<\/h2>\n\t<h4><strong>A Qualified Patient Must:<\/strong><\/h4>\n<ul>\n<li>Be a permanent or seasonal Florida resident<\/li>\n<li>Be diagnosed with a qualifying medical condition\u00a0by a qualified physician<\/li>\n<li>Be entered into the Medical Marijuana Use Registry<\/li>\n<li>Obtain a Medical Marijuana Use Registry Identification Card<\/li>\n<\/ul>\n\t<h2>Step 1<\/h2>\n\t<h3>Patients must first be Diagnosed with a Qualifying Medical Condition<\/h3>\n<p>Patients must visit and be treated by a &#8220;qualified physician&#8221; who has completed the required training and be diagnosed with a qualifying medical condition to qualify for medical marijuana.<\/p>\n<p>OMMU provides a <a href=\"https:\/\/knowthefactsmmj.com\/physicians\/list\/\" target=\"_blank\" rel=\"noopener\">list of qualified physicians in Florida<\/a>. Patients and caregivers can also <a href=\"https:\/\/mqa-internet.doh.state.fl.us\/MQASearchServices\/MedicalMarijuanaPhysician\">search for a qualified physician<\/a> by location and specialty.<\/p>\n<p>It is the responsibility of the qualified physician to follow Florida law, diagnose patients with a qualifying medical condition, and determine if medical marijuana is an appropriate treatment.<\/p>\n<p>OMMU recommends speaking to your health care professional to determine if medical marijuana products are right for you or your loved one.<\/p>\n<p>&nbsp;<\/p>\n<a href=\"https:\/\/appsmqa.doh.state.fl.us\/MQASearchServices\/MedicalMarijuanaPhysician\" class=\"wm-button size-medium button\" target=\"_blank\">Qualified Physician Search<\/a>\n\t<p><strong>Qualifying Medical Conditions:<\/strong><\/p>\n<table border=\"0\">\n<tbody>\n<tr>\n<td>\n<ul>\n<li>Cancer<\/li>\n<li>Epilepsy<\/li>\n<li>Glaucoma<\/li>\n<li>HIV\/AIDS<\/li>\n<li>Post-traumatic stress disorder (PTSD)<\/li>\n<li>Amyotrophic lateral sclerosis (ALS)<\/li>\n<li>Crohn&#8217;s disease<\/li>\n<li>Parkinson&#8217;s disease<\/li>\n<li>Multiple sclerosis (MS)<\/li>\n<\/ul>\n<\/td>\n<td>\n<ul>\n<li>Medical conditions of the same kind or class as or comparable to the others listed<\/li>\n<li>Terminal condition diagnosed by a physician other than the qualified physician issuing the physician certification<\/li>\n<li>Chronic nonmalignant pain caused by a qualifying medical condition, or that originates from a qualifying medical condition, and persists beyond the usual course of that qualifying medical condition<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n\t<h2>Step 2<\/h2>\n\t<h3>Patients and their Caregivers will be Entered into the Medical Marijuana Use Registry by their Qualified Physician<\/h3>\n<p>Be sure to provide your physician with your current email address so that you receive your login credentials to the Registry.
